What should be included in the consent document?

The treatment plan is explained in professional yet easy-to-understand language. Remember to mention all services that will be received today and any follow-up visits that are recommended (if needed).
Explain to the client and ensure that he/she understands and agrees.
This includes explaining the products and aftercare that will be given to the customer. Explain this to them and make sure they understand what needs to be done.
This is a statement that the customer has read and understands all of the above information. You must also confirm that you understand and agree to the service you are about to receive, that you have received all explanations and have had all questions answered.
Finally, have the client sign and date the form. Keep a copy for your records.
