A facial client consent form is a part of the pre-treatment process and are a necessary step before skin treatments can be performed.

These forms are usually part of a client acceptance form package or consultation to ensure that the client understands each skin treatment (and the entire service option), understands any risks, and has informed consent.

What should be included in the consent document?

While the fields on the different forms vary slightly depending on the services provided, each model cosmetic consent form should include the following.

Client Information

This will include the client's name, phone number, address, and the date and time of the appointment. You may also want to include emergency contact information if you wish.

Medical Information

It is important that each intake form include information about skin conditions, medical conditions, allergies, and other health issues (medical history). This is necessary for proper service selection and for liability purposes.

Skin Care Plan

Ask the client to note the current services and products used and provide a brief description of their current skin care regimen.

Skin Assessment

This can be done in two parts.

First, there is a space for the client to comment on their skin condition, preferences, skin care goals, and reported treatment history.

Second, there is a section for the professional to complete an evaluation of your skin condition, observations, and relevant medical information.

Proposed Treatment Plan

The treatment plan is explained in professional yet easy-to-understand language. Remember to mention all services that will be received today and any follow-up visits that are recommended (if needed).

Explain to the client and ensure that he/she understands and agrees.

Post-Treatment Instructions

This includes explaining the products and aftercare that will be given to the customer. Explain this to them and make sure they understand what needs to be done.

Check for Understanding

This is a statement that the customer has read and understands all of the above information. You must also confirm that you understand and agree to the service you are about to receive, that you have received all explanations and have had all questions answered.

Sign and Date the Form

Finally, have the client sign and date the form. Keep a copy for your records.
